What does this mean on an Exchange 2019 box?

As per more advice on Reddit, I had to run a patch released on the 2nd of Jan:

https://aka.ms/ResetScanEngineVersion 

It takes a few minutes and you need to make sure it completes in a dedicated Ex2019 Powershell window, but after that it seems to be back to normal.

Some people needed a server reboot but we didn't 🖐
