What does this mean on my disk RAID system?

Disk 04 is flashing red but the Avago software says it is fine...?

This has now moved on to:

Fatal firmware error Line 242 in ../../raid/mtask.c

After performing an upgrade to firmware 6.3 I am now seeing very slow performance and lots of these:
