Hello I am seeing the error:

disk has reached a logical block provisioning permanent resource exhaustion condition

Then the drive letter for the iSCSI connected QNAP NAS disappears

Help please!

This was due to thin provisioning being used which allowed more capacity to be allotted to storage than was physically available. The solution was to add more physical storage to allow the storage spaces to fill to their assigned limit.
